Annexure D
Declaration of Parent/Guardian for Minor Kids or Child Passport (both parents are giving consent)
Both the parents are giving consent, Sign the Annexure form and both the parents shall be present at the Passport Seva Kendra.
Note: One parent with the passports of both the parents shall be present at the time of issue of fresh passport to the minor children. (This is allowed only in case of Valid Reson only if one parent is not able to visit Passport Seva Kendra and final decision will depend on the passport issuing authority.)
Annexure C
Declaration of Parent/Guardian for Minor Passports (one parent not given consent)
Only one parent is giving consent, Sign the Annexure form and only one parent will be present at the Passport Seva Kendra.
This is applicable only due to the following one or more reasons:-
- When the Father/Mother of the Child passport application is travelling abroad/ overseas.
- When the Father/Mother is separated and no court case is pending before the court regarding divorce/marital dispute/custody of the child.
- When The father/mother has deserted (Missing) and the whereabouts are not known.
- When there is an ongoing court case for divorce/custody of the minor child and the court has not given any order prohibiting the issue of passport without the consent of father/mother.
- When there is a court order for the custody of the minor child with a parent who is applying for the passport.
- When the parents are judicially separated and custody of the minor child has not been defined in the court’s decree.

- Child Age upto 4 Years must submit 2 recent Passport Size Photo
- No need to submit Passport Size Photo as photo will be live captured in Passport Seva office.
Photo Guidelines
- Size of photo should be 4.5 cm x 3.5 cm.
- Background of photo should be plain white.
- 80-85% of photo area should be covered with face.
- Both Ears should be visible.
- Photograph should be of high quality.
- Photograph should be printed naturally with no editing done on it.
- Cap is not allowed.
- Photograph should be printed on professional photo paper.
